Optic pit maculopathy – diagnosis with spectral OCT and its implications for treatment

Objective: To present correlations between spectral OCT images of optic pit maculopathy with surgical appearance during vitrectomy.
Methods: 6 eyes with optic pit maculopathy were examined with both first and second generation of spectral OCT (Copernicus, Optopol, Poland) with axial resolution 6 um and 3 um respectively. In 4 eyes vitrectomy with intraocular gas were performed. In one eye temporary silicone oil was used. Follow-up with spectral OCT is 2 years.
Results: SOCT and surgical observation confirmed that cerebrospinal fluid may be coming through the optic nerve. Fluid initially enters inner retina layers causing a schisis, then intraretinal pulling forces produce photoreceptor layer detachment with pulling photoreceptor layer to its original position. Posterior hyaloid detachment by creation of additional fluid channel may be responsible for healing.
Conclusions: SOCT explains pathogenesis of optic pit maculopathy and confirms rationale of vitrectomy as treatment method.
